Informed Consent

This document contains important information regarding the holistic wellness services offered at Just Be Holistic Wellness Center. Please read this carefully so that you can make an informed decision. When you sign this document upon booking your first appointment, it will represent an agreement between you and Just Be Holistic Wellness Center. If you have any questions, please discuss them with your practitioner at any time.


Scope of Practice and Educational Qualifications


The staff at Just Be Holistic Wellness Center are not licensed physicians in Pennsylvania and do not possess a license to practice medicine from the state of Pennsylvania. Kari Eyer is the only licensed professional counselor at Just Be Holistic Wellness Center and no other staff members possess a license in mental health from the state of Pennsylvania. There are staff members licensed in Massage Therapy by the State of Pennsylvania.


The services offered at Just Be Holistic Wellness Center are complementary and, except for massage therapy and counseling, not licensed by the state of Pennsylvania. The services are offered to support rather than replace the care of your current physicians, therapists, acupuncturists, and other wellness or healthcare professionals. The services offered are designed to support your well-being.


Massage Therapy is “the application of a system of structured touch, pressure, movement, holding and treatment of the soft tissue manifestations of the human body in which the primary intent is to enhance the health and well-being of the client […] The term includes the external application of water, heat, cold, lubricants or other topical preparations, lymphatic techniques, myofascial release techniques and the use of electro-mechanical devices which mimic or enhance the action of the massage techniques.” (Massage Therapy Act 118 of 2008). Massage Therapists are not able to diagnose. To practice Massage Therapy in the state of Pennsylvania, a practitioner must have an active License in Massage Therapy from the state. There is a Student Massage therapist on staff, offering free massages until she is licensed.


Reiki, Energetic Tune Up, Mesa Blanca Ceremony, and Lemurian Light are energy techniques where the practitioner gently places their hands on the client's body to flow energy into and around the client. Hands off treatment is equally as effective and available upon request. These services are often used to support physical health, mental/emotional relaxation, and spiritual well-being. These sessions can be provided by certified Reiki practitioners, Shamans, Spiritists, and Lemurian Light Healers.


Ionic Foot Detox uses a warm foot soak and electricity to create a chemical reaction that supports the release of toxins out of the body through the feet. Clients often describe this treatment as relaxing and calming. An Ionic Foot Detox can be offered without official training or certification. Just Be Holistic Wellness Center has a remote consulting physician to inform treatment protocols, as requested.


John Barnes Myofascial Release Treatment® is a structural approach to releasing tension and promoting physical well-being. This treatment involves passive, gentle stretching. The minimum requirement for a practitioner to offer this service is to be certified in Myofascial Release 1 in the The John Barnes' Myofascial Release Approach®.


Reflexology is the stimulation of the reflex points in the feet to promote physical and emotional well-being, especially of specific organs, body structures, and chakras. Reflexology can be offered by certified Reflexologists and Licensed Massage Therapists.


Acupressure is the stimulation of acupressure points and meridians throughout the body to support the release of energetic stagnation, which may support physical health, relaxation, and energetic balance. Acupressure can be offered by Licensed Massage Therapists.


Realization Process™ is a style of meditation is designed to help you connect with the internal space of your body to experience wholeness and support a gentle release of emotions, thoughts, and physical tension. The Realization Process is offered by a certified Realization Process™ Teacher.


Medical Qigong is a form of Chinese Medicine that recognizes root causes of symptoms or disease by addressing energetic imbalances or blockages that contribute to illness. Medical Qigong is offered by a certified Medical Qigong Practitioner.


Intuitive Readings are conducted by mediums to intuit spiritual messages that feel aligned with the frequency of the client's energy. Intuitive Readings include Meet Your Spirit Council, Intuition Training, Past Life Reading, Tarot Card Reading, and Medium Reading. Intuitive readings can be offered by mediums, spiritists, Shamans, and Reiki practitioners.


A Healy is a microcurrent frequency device, approved by the FDA. It can send hundreds of frequencies through your body in minutes to address imbalances, deficiencies, and pain. Healy sessions are not suitable for people who have pacemakers, implanted defibrillators, foreign metal objects within the body, history of radiotherapy treatment, sensory disorders, epilepsy, heart disease, fever, or are pregnant. A Healy session can be offered by anyone who has received sufficient training in the use of the device.


Complex Trauma Recovery Coaching is designed to provide clients with coping strategies to support the recovery from Complex Trauma, a trauma often experienced during childhood. Shelby is currently enrolled in a certification program, has not completed the program, and has express permission to practice Complex Trauma Recovery Coaching techniques prior to certification. While Shelby is a student, Complex Trauma Recovery Coaching sessions include an intake, evaluation of wellness goals, demonstration of a coping technique, and Reiki. Complex Trauma Recovery Coaching can be offered by Certified Complex Trauma Recovery Coaches or students of an accredited program by the International Complex Trauma Association.


Confidentiality and Limitations to Confidentiality


All information discussed or written within a consultation or treatment will be kept strictly confidential by the practitioner. Release of any information to persons outside of Just Be Holistic Wellness Center may occur under the following conditions: written consent by you; by order of the court; as required by state and/or federal law; suspected child or elder abuse/neglect; or situations in which you may exhibit evidence of being a danger to yourself or others.


What You Can Expect


Sessions at Just Be Holistic Wellness Center begin with a discussion regarding your wellness goals, any symptoms, and your experience with implementing relaxation techniques, coping strategies, or energy management tools, as expressed by your practitioner. All services except massage are performed with the client fully clothed; during a massage, you are permitted to undress to your comfort level. Most services include various touch and non-touch techniques. If you experience any discomfort or unwanted touch, please notify your practitioner, who will make adjustments and offer support.


The purpose of the services offered at Just Be Holistic Wellness Center is to promote overall well-being, including physical, emotional, mental, energetic, and spiritual. There is no guarantee that the services offered will be helpful. Wellness goals are best achieved when both the client and practitioner are actively involved in feedback about treatment results, setting new goals, and establishing lifestyle changes.


The outcome of your treatment depends largely on your willingness to engage in the treatment, which may, at times, result in discomfort. Because the services offered involve connecting with physical tension, unresolved emotional content, and energetic stagnation, you may experience physical soreness, tightness, unpleasant emotions, and active or unpleasant dreams. These experiences can bring on strong feelings of anger, depression, anxiety, sadness, etc. Because there are no miracle cures, your practitioner cannot guarantee your symptoms or concerns will change. Your practitioner will support you, including trying to better understand your condition, needs, wants, and how to support your achievement of those goals.


Client Responsibilities


To receive the most benefit from a holistic wellness treatment, it is essential that you attend your sessions and make an effort to be honest and open about issues that you are facing, including any issues that arise between you and your practitioner. Becoming upset or disappointed by a practitioner can be a result of addressing unresolved emotional content. It may be beneficial to address these concerns with your practitioner as they arise as a way of meeting your wellness goals. Becoming upset or disappointed by a practitioner due to improper conduct should be addressed with the business owner.


At Will Participation


Your participation in services at Just Be Holistic Wellness Center is “at will.” This means that you may choose to end your participation at any time by notifying the practitioner with whom you are scheduled. Compensation of ending a treatment early is not guaranteed.


Other Rights


You have the right to considerate, safe, and respectful care, without discrimination as to race, ethnicity, color, gender, sexual orientation, age, religion, or national origin. You have the right to ask questions about any aspect of your treatment and your practitioners' training and experience. The relationship between a client and practitioner is strictly professional and involves appropriate boundaries that must be maintained by all involved. A client-practitioner relationship is important and cannot involve any kind of romantic relationship. Additionally, a client-practitioner relationship cannot involve violence, which includes verbal or physical threats to anyone present. Your practitioner reserves the right to terminate treatment immediately and notify the police.


Payment


Payments are accepted via cash, check, or credit/debit card. Returned checks are subject to an additional $40.00 fee. If you refuse or are unable to pay, you will be denied further services until all debts are paid in full. If you refuse to pay your debt, Just Be Holistic Wellness Center reserves the right to collect the debt according to the guidelines established by the federal government in the Fair Debt Collection Practices Act. If fees are adjusted in the future, a 30-day notification will be provided in advance.

Just Be Holistic Wellness Center is located at 519 Main Street, Suite A, Royersford, PA 19468. Office hours are by appointment only.

To contact your practitioner, you can utilize the messaging feature on the Staff page of our website or any methods of communication established between you and your practitioner.


To contact the business owner, you can call or text (484) 302 - 2234 or email hello@justbeholisticwellness.com. The business owner is unavailable for immediate communication, including phone call, when she is with clients or otherwise unavailable. You may leave a voicemail, but missed calls without a voicemail will not be responded to. Any communication with the business owner will be responded to as soon as possible and may take up to 1 business day.


Emergencies and Immediate Assistance


If you are experiencing an emergency and/or you feel unable to keep yourself safe, 1) call 9-1-1, 2) go to the Emergency Department at your Local Hospital, or 3) contact Service Access Management for Crisis Services at 1-877-236-4600.


Consent


While booking your first appointment, you will be asked to sign and affirm that you have read, understand, and agree to the information contained in this document regarding your participation in services at Just Be Holistic Wellness Center.
